Janice  Kohn,  MSW is a senior‑living specialist and social worker who has spent her career helping older adults and their families navigate the full spectrum of housing and care options — from Assisted  Living and Memory  Support to Life‑Plan (CCRC) communities. Drawing on deep relationships with hospitals, rehabilitation centers, and post‑acute providers, she orchestrates seamless transitions at discharge, confirming that medications, durable medical equipment, and follow‑up appointments are in place before a client ever crosses the threshold of their new home.

Known for attentive listening and clear, compassionate guidance, Janice excels at translating each client’s medical, emotional and financial priorities into a living solution that feels both safe and empowering.
